Evening Program
Evening Program Now Available for Dementia Patients and Sundowners

Offered on Wednesdays from 4 pm to 9 pm at Alzheimer’s Family Center, our Evening Program offers evening respite for caregivers of Alzheimer patients. The program can also address the symptoms of patients suffering from sundowning syndrome.

What does the program include for your loved one?
The evening program offers the same high-quality services as our weekday program; evening memory care for seniors includes:

Physical and occupational therapy
Massage and spa
Nutritious dinner
Activities and games
All patients are under the supervision of a nurse and a social worker.
Transportation to and from the center is free of charge

A Social Worker will discuss all funding options and work with you to enroll your loved one.

Caregiver Support Groups
Attend a caregiver support group while your loved one attends the Evening Program! Share experiences, receive encouragement, and problem-solve life challenges — and feel understood. Support groups are facilitated by AFC’s Masters level clinicians and are a proven way for caregivers to build knowledge and skills while receiving support from peers and professionals alike. Support groups are held on the second and fourth Wednesdays of each month from 6:00 pm to 7:30 pm. 

Adult Evening Health Care services at Alzheimer’s Family Center are made possible by a grant from Archstone Foundation.

Social Work

  • Case management

  • Individualized education on stages of dementia
  • Early-stage support group led by Masters level clinician

Personal Care

  • Personal assistance geared toward patient needs
  • Incontinence care
  • Collaboration with each patient’s physician to ensure optimal care.


  • Planned schedule of independent and group activities
  • Exercise, games and creative arts
  • Pet therapy and gardening
  • Dakim brain fitness computers
  • Music Therapy


  • A balanced dinner meeting nutritional standards for older adults
  • Nutritional guidance for special diets, maintaining healthy weight, etc.


  • Physical, occupational and speech therapy

Mental Health

  • Consultation with licensed psychiatrist, psychologist or licensed clinical social worker
  • Individual, couples and family counseling services*

Medical Supervision

  • Oversight by medical director and pharmacist
  • Collaboration with each patient’s physician to ensure optimal care.

Care Planning & Ongoing Assessments

  • Individual plan of care is updated every six months based on multidisciplinary patient assessments


  • Transportation coordination services
