people with memory loss — and their families.
Our adult day health care services and adult day program give people coping with memory loss tools, resources and programs that will sustain them through the long haul – preparing the family to make the best of each day, creating happier, healthier moments along the way.
Get ready for days focused on hope, health and happiness.
Enrolling your loved one in a day care for elderly with dementia program is a big step. So we begin with a complete assessment of the patient’s state of mental and physical health, and design a program that balances the right amount of stimulation, challenge and tender loving care. Following the evaluation, we will recommend the early stage program – which we call the New Connections Club – or the mild-to-late stage program, our Friendship Club.
Something for almost everyone.
Our professional staff, comprehensive programs and cutting-edge facility make it possible for AFC to care for people from the earliest to most advanced stages of dementia, including those suffering from depression and memory loss and grief and Alzheimer’s. We can often welcome patients who’ve been discharged from other programs for behavior-related challenges such as wandering.
And we speak your language: AFC programs and services are offered in English, Spanish and Vietnamese. (Many other languages are available if needed.)
Get started on a brighter future.
